Guardian EMS may collect information that you voluntarily provide through contact, scheduling, billing, recruiting, or other website forms. Information should be used only for the purpose for which it was submitted, operational follow-up, service administration, legal obligations, security, and other legitimate business purposes.
Do not use general website forms to submit information that is not requested. Transport requests may involve health-related or identifying information and should only be accepted through systems Guardian EMS has approved for that purpose.
Third-party services used for website hosting, analytics, communications, payment processing, recruiting, or scheduling may process information according to their own terms and privacy practices.

When you use the Guardian EMS Trip Calculator or submit a transportation request, we may collect information such as the patient name, date of birth, callback information, requested transport date and time, trip type, pickup and destination addresses, route coordinates when an address is verified, mileage, and required eligibility acknowledgments.
Guardian EMS uses this information to review the request, determine whether the requested service is appropriate, coordinate transportation, communicate about the request, support billing or operational records when applicable, and protect the security and integrity of the website.
Do not use the website to submit unrelated medical records or detailed clinical information. Online forms are not for emergencies. Call 911 for a medical emergency.
When you ask the Trip Calculator to verify an address or calculate road mileage, Guardian EMS may send the address or geographic coordinates to third-party mapping and routing services through our server to return a standardized location and route estimate. Do not enter unrelated medical details into address fields.
These tools support trip planning only. Dispatch may use a different route or mileage when reviewing or completing the actual transportation request.
